Waupaca Area Triathlon
Volunteer Waiver and Release From Liability
It is the understanding of the Waupaca Area Triathlon that I, my estate or heirs and any other person who may have a claim as a result of my death, injury or disability hereby FOREVER RELEASE, DISCHARGE AND COVENANT NOT TO SUE THE WAUPACA AREA TRIATHLON directors, insurers and agents from any and all liabilities, claims, demands, or causes of action that may hereafter have for injuries or damages arising out of my participation as a volunteer in the 2007 Waupaca Area Triathlon.
I acknowledge and understand that should I incur an injury requiring medical attention as a result of my participating as a volunteer the Waupaca Area Triathlon volunteer liability coverage for me shall be a secondary payee policy and as such my own personal medical coverage shall be the primary payee.
This waiver is effective for the 2007 Waupaca Area Triathlon event.
